Epilepsy has long been treated using voltage-gated sodium channel blockers such as phenytoin and carbamazepine. Many modern second-generation (fosphenytoin, oxcarbazepine, lamotrigine, felbamate, topiramate, zonisamide) and third-generation antiepileptic medicines (eslicarbazepine, brivaracetam, carisbamate, fluorofelbamate, elpetrigine, lacosamide, rufinamide, safinamide, vinpocetine) (AEDs). When it comes to therapeutic decisions, clinicians are faced with a profusion of possibilities. Only after thorough randomized controlled studies have novel antiepileptic medicines (AEDs) or devices been approved for sale. Add-on trials, in that patients with refractory epilepsy who are on stable AED therapy, are randomized to receive a study drug or a placebo, are the most prevalent designs for initial AED efficacy trials. This method can be criticized. Patients with refractory epilepsy make up a small percentage of the epilepsy community, hence the findings may not be applicable to the general population. Furthermore, partial epilepsy is not a one-size-fits-all condition. In add-on studies, drug interactions can be a difficult confounding factor.
